Role Overview

The Community Support Worker position is a permanent full-time role (39 hours weekly) based at the Elevate Service in Sligo. The role involves delivering personalised community support to individuals with intellectual, physical, and mental health challenges, aiding their integration and active participation in local communities. This includes enhancing their emotional, social, occupational, and learning opportunities through various programs.

Responsibilities

  • Design, implement, monitor and assess educational, recreational, and occupational programs within the community following best practices aligned with Quality of Life improvements.
  • Collaborate with families, multidisciplinary teams, and community personnel to facilitate accessible community engagements for service users.
  • Maintain accurate documentation, update records, and complete necessary paperwork to ensure accountability and effective communication.
  • Engage with service users respectfully, promoting dignity, equality, and choice in all interactions.
  • Provide personal care assistance and support everyday living skills ensuring users’ comfort and hygiene.
  • Promote independent living by encouraging service user integration into the local community and access to resources.
  • Maintain privacy and confidentiality of service users at all times.
  • Adhere to health and safety regulations, managing safe appliance and equipment use, and follow established risk assessments.

Requirements

  • Desirable minimum qualification of QQI Level 5 in Health and Social Care or equivalent.
  • Willingness to engage in Rehab Care’s training programs and other relevant external training opportunities.
  • Proven experience supporting individuals with intellectual, physical, and mental health challenges.
  • Demonstrated success in facilitating meaningful community experiences and promoting self-advocacy among service users.
  • Experience delivering personal care and supporting community independence.
  • Possession of a valid driving licence and access to own transport is essential.
  • Candidates may be required to provide police clearance for all countries resided in prior to moving to Ireland.

Additional Information

A recruitment panel may be formed following this campaign. The salary scale ranges from €31,561.56 (point 1) to €35,982.43 (point 5) pro rata per annum, with progression up to €43,731.06 based on a 12-point scale reflecting qualifications and experience.
