Job description
Role Overview
This full-time, one-year fixed term position based in Limerick involves contributing to a Housing First Case Management Service designed to assist individuals facing prolonged homelessness in Limerick City by employing the Housing First approach.
Key Responsibilities
- Implement the Housing First model's principles, emphasizing client autonomy, choice, and independence.
- Create a secure, inviting, and supportive environment for people who are homeless, at risk of homelessness, or residing in temporary housing.
- Establish and nurture professional, trust-based relationships to aid clients in achieving stability and sustainable independent living.
- Conduct assessments and develop care and support plans using specialized case management and outcome measurement tools.
- Assist clients in securing and maintaining suitable accommodation, as well as accessing welfare benefits and move-on housing options.
- Support tenants in tenancy management, including life skills development, rent payment, maintenance reporting, and good neighborly conduct.
- Maintain and strengthen partnerships with social housing providers, private landlords, and relevant service agencies.
- Advocate for clients during appointments with healthcare, counseling, legal, social, and other support services as needed.
- Collaborate within multidisciplinary teams and coordinate with statutory, voluntary, and community agencies.
- Offer guidance to prevent homelessness and help clients access educational, training, employment, and community resources.
- Manage an active case load with accurate documentation and timely database updates using organizational systems.
- Participate in supervision, team meetings, trainings, and contribute to ongoing service development and best practices.
- Work adaptively to meet service demands, maintain safety standards, support volunteers or students, and fulfill additional assigned duties.
Required Qualifications and Experience
- A degree-level qualification in social care or related fields such as youth work, social studies, addiction, or psychology; alternatively, registration as a Social Care Worker with CORU.
- Previous experience working with individuals who are or have experienced homelessness.
- Experience supporting clients with dual diagnoses.
- Demonstrated ability to work effectively within teams.
- Competent in report writing.
- Experience collaborating with various agencies.
Desirable Attributes
- Familiarity with homelessness issues and the challenges faced by people affected.
- Possession of a full driving license.
