Role Overview
The Transition Support Officer (TSO) plays a vital role in providing direct, practical assistance, training, and guidance to clients experiencing various transitions such as from school to community hubs, between different community or regional hubs, or moving between residential and community care. Stationed at MINDS Regional or Community Hubs, the TSO aids the Hub Manager in collaborating with a multidisciplinary team comprising Allied Health Professionals, Training Officers, Vocational Coaches, and Learning Support Trainers to develop and regularly review individualized development plans (IDPs). The TSO also partners with Social Workers, Care Coordinators, and Training Officers to engage caregivers effectively, ensuring transfer of skills and regular updates on client progress.
Key Responsibilities
- Assist the Hub Manager in drafting, reviewing, and updating client IDPs using a person-centred methodology.
- Coordinate and actively participate in transition planning, case reviews, and IDP meetings involving AHPs and other stakeholders.
- Ensure clients advance on their IDP goals, which align with their long-term aspirations and interests.
- Engage clients and caregivers in discussions regarding transitions and training needs with Social Workers as appropriate.
- Organize and attend family meetings to review operational plans and broader person-centred objectives.
- Deliver hands-on training tailored to clients' transition requirements across different settings and document progress meticulously.
- Advocate for clients' transitional and development needs in collaboration with multidisciplinary teams, caregivers, volunteers, and community partners focusing on environmental, socio-emotional, and behavioral supports.
- Maintain detailed records of stakeholder engagements and training adaptations.
- Accumulate and share effective person-centred practices and tools to inform and support fellow practitioners.
- Conduct assessments of clients' transition and developmental needs, monitoring progress through pre- and post-intervention evaluations.
- Collaborate with teams to revise and adapt IDP goals if outcomes are unmet or to elevate clients' independence levels.
- Support the Transition Coordinator in documenting client outcomes and preparing consolidated reports on overall IDP achievements.
Qualifications and Skills
- Possess at least a Diploma in Social Work, Psychology, Community Services, or related field with a minimum of two years' relevant work experience.
- Demonstrate strong teamwork abilities and excellent interpersonal skills.
- Exhibit robust organizational, time management, and problem-solving capabilities.
- Communicate effectively both verbally and in writing; proficiency in an additional local language besides English is considered an advantage.
- Be self-driven, adaptable, and capable of working both independently and collaboratively with integrity, sensitivity, and dedication.
- Show a genuine passion for working with people who have intellectual disabilities.
