Job description
Overview
We are inviting applications for the position of Service Coordinator to join the Enliven team based in Tauranga. This role focuses on providing high-quality, person-focused home and community support services. Reporting to the Lead Service Coordinator, the incumbent will oversee flexible, restorative services centered on the needs and priorities of the people supported. The position requires collaboration with service users, families, support workers, allied health professionals, and external partners to coordinate services effectively and ensure compliance with Enliven's model and contractual obligations.
Key Responsibilities
- Ensure timely and appropriate support for service users through effective service coordination
- Conduct clinical assessments and reviews for clients with non-complex needs
- Create and maintain personalized support plans that promote independence and wellbeing
- Assist with rostering and scheduling, ensuring suitable matching between support workers and service users
- Develop and nurture strong partnerships with NASC, DHBs, GPs, and community organizations
- Participate in reporting, quality assurance, and initiatives aimed at service enhancements
- Support staff development and foster a positive team environment
- Practice in a culturally respectful manner that recognizes Tangata Whenua
Candidate Profile
- Registered Nurse, Physiotherapist, or Occupational Therapist qualification is essential
- Possession of a full New Zealand driver's license without restrictions
- Experience in aged care, community health, or rehabilitation environments
- Proven skills in assessment, documentation, and care planning
- Background in coordinating services and collaborating with multidisciplinary teams
- Ability to prioritize and manage competing demands in a dynamic workplace
- Strong communication and problem-solving capabilities
