Residential Care Case Manager

About The Role

The Residential Care Case Manager is responsible for receiving referrals, conducting assessments, and coordinating intake arrangements for residential care placements in collaboration with the Case Manager Coordinator. This role involves developing and implementing a Looking After Children (LAC) Care Plan tailored for each child in the caseload, continuously monitoring placements against the LAC Care Plan and the Department of Health and Human Services (DHHS) Best Interests Plan, and maintaining a current understanding of each child’s or young person’s development.

This full-time position is based in Geelong, Victoria, as part of the Out of Home Care Program, reporting directly to the Case Manager Coordinator.

Key Selection Criteria

• Strong knowledge and appreciation of Aboriginal culture and values, with an understanding of challenges faced by Aboriginal children, youth, and their families.
• Relevant tertiary qualification in social work, community services, or an equivalent discipline.
• Comprehensive professional understanding of trauma, attachment theory, and skills in integrated trauma-informed practice.
• Experience working in child welfare, particularly relating to addressing children's developmental needs.
• Familiarity with child safety principles and standards to ensure protective practices.

Requirements

• Valid and current driver's license.
• Successful criminal history check funded by the employer.
• Current and valid Employee Working with Children Check.
• Current NDIS Worker Screening Check for client-facing or risk-involved roles (support offered for application process).
