Job Overview
This full-time position centers around working alongside a multidisciplinary team to create and implement release strategies for patients transitioning from correctional environments.
Key Responsibilities
- Arrange and coordinate behavioral and mental health services, medical care, and community support resources required post-release.
Qualifications and Requirements
- Bachelor's degree in Psychology, Social Work, Counseling, or a similar discipline.
- Preferred minimum of two years of experience in release planning.
- Background in correctional healthcare, criminal justice, or forensic settings is advantageous.
- Must successfully complete background screenings and secure required security clearance.
- Certification in Basic Life Support (BLS) is mandatory.
- Demonstrated excellent communication skills and effective collaboration abilities.
- Familiarity with community resource networks.
