Job Overview

Seeking an experienced Home Care Worker for full-time, permanent employment across Sydney’s North Shore and Northern Beaches. The role involves supporting older adults who are returning home post-hospitalization, assisting them in rehabilitation and regaining independence within their own residence. Work hours are Monday to Friday, 9:00 am to 5:00 pm, totaling 76 hours per fortnight.

Key Responsibilities

• Deliver respectful, individualized personal care tailored to client needs.
• Assist clients with exercise regimens as prescribed by allied health professionals.
• Provide support with compression garments and advanced care requirements.
• Transport clients to medical or other appointments.
• Offer social interaction and meaningful engagement.
• Help with meal preparation tasks.
• Provide domestic support and other practical assistance within the home.
• Monitor wellbeing of clients and promptly report any changes or concerns.
• Keep accurate and up-to-date documentation of all services rendered.

Candidate Profile

• Holds at least a Certificate III in Individual Support, Aged Care, or related fields.
• Minimum six months experience in home care, aged care, or a similar setting.
• Previous exposure to clients with post-hospital or higher care needs is highly desirable.
• Possession of a valid Australian driver’s license.
• Access to a reliable vehicle with comprehensive insurance coverage.
• Strong skills in communication and documentation.
• Ability to comply with all mandatory screening and vaccination protocols.
