HCC Certified Medical Coder - Chennai

About the Role

Access Healthcare is seeking an HCC Certified Medical Coder to join their team in Chennai. The candidate will be responsible for reviewing medical records, accurately assigning diagnosis codes, and mapping those to Hierarchical Condition Categories to support revenue cycle management. The role involves auditing medical documentation to ensure precision and compliance with coding standards.

Responsibilities

• Examine patient medical records to determine diagnoses and treatment details.
• Assign ICD-10-CM diagnosis codes to patient records effectively.
• Map diagnoses to Hierarchical Condition Categories (HCC) for proper classification.
• Maintain consistency and accuracy in code assignments across records.
• Conduct audits on records and documentation to ensure accuracy.
• Provide constructive feedback to physicians and healthcare providers regarding coding standards and guidelines.
• Educate physicians, providers, and clinical staff on proper clinical documentation compliance.

Requirements

• Demonstrated experience or background in medical coding practices.
• Strong attention to detail with a commitment to accuracy and adherence to compliance standards.
• Proficiency with coding systems including ICD-10-CM, CPT, HCPCS, and HEDIS CAT II codes.
• Possession of at least one certification among CRC, CPC, or CCS is mandatory.
