Role Overview

As a Backend Engineer specializing in Healthcare Integrations, you will develop and maintain the integration infrastructure connecting FluidAI’s data-rich platform with hospital systems such as electronic health records (EHR), laboratory information systems (LIS), device gateways, and national health data exchanges. This role involves crafting robust production APIs and services compliant with healthcare interoperability standards, ensuring reliable, secure data exchange in clinical environments. Regular travel within the Middle East to partner with internal teams, hospital IT departments, and clients is expected.

Key Responsibilities

  • Design and sustain integration services facilitating clinical data exchange between FluidAI’s platform and hospital systems including EHRs, LIS, ADT feeds, device gateways, and national health information networks.
  • Develop and uphold HL7 FHIR APIs and resources (Patient, Encounter, Observation, Device, DiagnosticReport) alongside HL7 v2 message interfaces (ADT, ORU, ORM) for both inbound and outbound data transactions.
  • Build reliable, well-tested backend services and REST or event-driven APIs optimized for security, observability, and resilience in hospital network environments.
  • Manage data translation and mapping between internal data structures and external standards, including terminology frameworks such as LOINC, SNOMED CT, and UCUM.
  • Implement robust interface engines and integration patterns covering queuing, retry strategies, deduplication, acknowledgments, idempotency, and reconciliation.
  • Lead technical aspects of hospital onboarding by gathering interface specifications, performing connectivity tests, and supporting deployment and initial operational phases.
  • Set up monitoring, alerting, and logging mechanisms to ensure early detection and resolution of message failures and data inconsistencies.
  • Adhere to healthcare security and privacy protocols including OAuth2/SMART on FHIR, mutual TLS, VPN, audit logging, and data minimization aligned with regional laws such as Saudi NPHIES and international frameworks like HIPAA and PIPEDA.
  • Generate and maintain comprehensive technical documentation including interface specifications, FHIR implementation guides, integration runbooks, and data flow diagrams.
  • Work collaboratively with product, clinical, regulatory, and quality assurance teams to ensure integrations support clinical workflows and comply with medical software standards (IEC 62304, ISO 13485).
  • Participate actively in Agile development practices including sprint planning and roadmap discussions, focusing on integration and platform priorities.
  • Engage closely with software, hardware, data science, and machine learning teams to provide dependable data services spanning the full technology stack.

Candidate Profile

  • Proven experience as a Backend, Integration, or Interoperability Engineer, preferably in healthcare or medical technology sectors.
  • Extensive practical knowledge of HL7 FHIR (R4/R5), including resource modeling, profile extensions, search functionality, bulk data handling, and working with FHIR servers or SDKs such as HAPI FHIR or Firely.
  • Strong hands-on experience with HL7 v2.x messaging structure, segments, acknowledgment handling, and clinical workflows (ADT, ORU, ORM, MDM).
  • Familiarity with interface engines like Mirth Connect, Rhapsody, InterSystems, Cloverleaf or similar platforms.
  • Expertise in at least one backend programming language—Python, Java/Kotlin, C#/.NET, Go, or TypeScript/Node.js—with ability to design well-structured APIs (REST, OpenAPI, webhooks) and knowledge of message brokers (Kafka, RabbitMQ, MQTT).
  • Advanced skills in SQL and data modeling with relational databases (PostgreSQL, SQL Server, Oracle) and exposure to NoSQL/time-series databases.
  • Experience with deployment and operations in cloud and hybrid environments including AWS, Azure, or GCP, containerization with Docker and Kubernetes, CI/CD pipelines, and infrastructure as code; including on-premise hospital deployments.
  • Working knowledge of healthcare security and identity protocols such as OAuth2/OIDC, SMART on FHIR, TLS/mTLS, encryption standards, and auditing.
  • Preferred experience integrating with EHR vendors (Epic, Oracle Health/Cerner, MEDITECH, InterSystems TrakCare) and collaborating directly with hospital IT teams.
  • Knowledge of clinical terminologies (LOINC, SNOMED CT, ICD-10, UCUM) and medical device connectivity standards (IEEE 11073, IHE profiles) is advantageous.
  • Understanding of regulated software development processes (IEC 62304, ISO 13485) including risk management and traceability is a plus.
  • Fluency in English is required; Arabic language skills are strongly preferred.
  • Willingness and ability to travel within Saudi Arabia, including extended stays, is essential.
  • Capacity to thrive in fast-moving, multidisciplinary teams and environments emphasizing collaboration.
  • A proactive problem-solving approach with a focus on impact, ownership, and continuous enhancement.
