Key Responsibilities

• Ensure the safety of residents during transportation to and from medical appointments.
• Accompany residents throughout their medical visits and remain until the visit concludes.
• Follow specific protocols during visits, including signing in residents, presenting documents to medical staff, obtaining prescriptions and orders, scheduling follow-ups, and reporting visit details to the Resource Care Manager.
• Perform additional duties as assigned.

Candidate Requirements

• Physically capable of assisting both ambulatory and non-ambulatory residents with transfers between wheelchair and vehicle.
• Possess a valid state driver’s license with a clean driving record.
• Certified Nurse Assistant (C.N.A.) qualification required.
• Clean criminal background check.
• Hold any additional certifications mandated by the state.
