Job description
Role Overview
The Case Manager position entails directing and supervising personalized care strategies for clients, ensuring consistent support across clinical, psychological, and social service domains. This full-time onsite role based in Greater Coimbatore demands conducting intake evaluations, compiling comprehensive client histories, and collaborating with psychiatrists, psychologists, therapists, and other team members to establish and supervise treatment objectives.
Key Responsibilities
- Perform intake assessments and collect client backgrounds to tailor care plans.
- Coordinate with multidisciplinary professionals to develop and oversee treatment goals.
- Maintain precise, up-to-date client documentation and monitor client progress vigilantly.
- Adjust care plans as required to facilitate sustained recovery and community reintegration.
- Facilitate communication among clients, their families, and the treatment team.
- Organize family meetings and follow-up sessions to support client welfare.
- Link clients with local community resources and aftercare services for ongoing support.
- Advocate for clients’ needs while fostering a secure, supportive treatment environment.
- Ensure adherence to ethical guidelines and confidentiality in all interactions.
Qualifications and Requirements
- Strong expertise in case management encompassing assessment, care planning, coordination, and follow-ups across various disciplines.
- Comprehensive understanding of mental health, addiction recovery, and psychosocial rehabilitation, with empathetic engagement of diverse clients.
- Exceptional communication and interpersonal skills to interact effectively with clients, families, and professionals, including skills in conflict resolution and counseling.
- Proficient organizational skills for maintaining detailed records, utilizing digital systems, and upholding compliance and confidentiality protocols.
- Educational background: Bachelor's or master’s degree in Social Work, Psychology, Counseling, or related disciplines.
- Additional certification or training in addiction counseling or psychiatric social work is advantageous.
- Prior experience within residential rehabilitation, mental health services, or hospital settings is highly preferred.
- Capacity to work onsite at Sowripalayam, Coimbatore, manage multiple cases efficiently, and function effectively within an evidence-based multidisciplinary team.
- Fluency in Hindi is required.
