The Institute of Mental Health (IMH) in Singapore, founded in 1928, is a leading psychiatric hospital with a 2,000-bed capacity located in Buangkok Green Medical Park. IMH delivers comprehensive mental health treatment through 50 inpatient wards and 7 outpatient clinics. The institution also contributes significantly to the education of mental health professionals via the NHG-AHPL Residency Programme and partnerships with local educational bodies.
Role Summary
The Case Manager is responsible for delivering vital case management services to persons affected by mental health issues.
Key Responsibilities
- Conduct comprehensive assessments to evaluate client needs, medical conditions, and social circumstances.
- Design personalized treatment plans, arrange necessary services, and provide continuous support to aid client recovery.
- Work collaboratively with clients, their families, and multidisciplinary teams to formulate and implement intervention strategies.
- Manage crisis situations by performing risk evaluations, developing safety plans, and organizing urgent care to ensure client stability.
- Participate in quality assurance and departmental functions to enhance service delivery.
Qualifications
- Holds a diploma or degree qualification.
- Background in nursing or allied health, including clinical experience in psychiatry.
