Role Overview
The Support Coordinator role is full-time and primarily hybrid based in Central Coast, NSW, allowing for flexible work-from-home options. The incumbent will support NDIS participants by helping them comprehend their plans, connect with suitable services, and coordinate supports across multiple providers.
Key Responsibilities
- Develop tailored support plans aligned with participant needs.
- Manage and coordinate bookings for various services.
- Monitor participant progress toward their set goals.
- Address and resolve challenges or barriers affecting service delivery.
- Communicate regularly with participants, families, and diverse external stakeholders.
- Maintain thorough and accurate records consistent with NDIS guidelines and organisational policies.
- Advocate for participant needs and provide guidance about available resources.
- Contribute to ongoing enhancements of support coordination processes.
Qualifications and Skills
- Strong interpersonal capabilities to build respectful and collaborative relationships with participants, their families, and service providers.
- Clear and effective communication skills, both written and verbal, suitable for a diverse audience.
- Experience in case management involving planning, service coordination, and progress monitoring for individuals with complex needs.
- Customer service orientation emphasizing empathy, responsiveness, and effective problem-solving.
- Basic proficiency with digital tools, client management systems, and online NDIS platforms.
- Sound understanding of the NDIS scheme, disability services sector, and principles of person-centred practice.
- Ability to independently handle multiple priorities while maintaining accurate documentation in a hybrid working model.
- Relevant certification or diploma in community services, social work, disability support, or a related discipline, or equivalent practical experience.
- Possession of a valid driver's license and readiness to travel across Central Coast and Lake Macquarie as necessary.
