Social Worker

Overview

At Extendicare Eaux Claires, a long-term care facility with 204 residents, the Registered Social Worker delivers psychosocial support to residents and their families. This role involves assessing emotional and social needs and crafting personalized care plans to improve residents' quality of life. Collaboration with healthcare professionals, counseling facilitation, discharge planning support, and connecting residents to community resources are key aspects of the role. Strong communication, empathy, and knowledge of aging and healthcare systems are essential.

Core Responsibilities

• Address and respond suitably to inquiries relating to placement, lifestyle, policies, and procedures at Extendicare Inc.
• Lead tours of the facility for applicants, prospective residents, and their advocates.
• Conduct pre-admission procedures, including application screening, acknowledgment correspondence, pre-admission visits, assessments, and communication with placement agencies.
• Coordinate new admissions, supporting residents and families/advocates throughout the admission process.
• Advocate for residents and families to promote a resident-focused environment.
• Plan, execute, and manage social work programs to support the ongoing psychological needs of residents and their families.
• Participate as part of the multidisciplinary team in overall resident care planning and development of programs based on resident requirements.
• Encourage and advise the creation and functioning of resident groups where appropriate.
• Organize and engage in resident care conferences and family meetings as necessary.
• Develop and conduct regular family orientation programs for new and prospective residents' families/advocates.
• Provide educational and consultative services on social work topics through staff orientation, development programs, and seminars targeting staff, residents, and families.
• Maintain documentation and produce statistical reports to support quality management, evaluate departmental performance, recognize trends in resident and social needs, and guide policy and program planning.
• Work responsibly and ethically in a position of trust with vulnerable adults (residents).
• Uphold the organization's values related to ethics, morality, and integrity as outlined in Extendicare's Code of Conduct.
• Perform additional duties as assigned.
