Key Duties

• Deliver comprehensive rehabilitation support to individuals in their own communities.
• Collaborate within multidisciplinary teams to aid clients in accomplishing their rehabilitation objectives.
• Navigate and apply knowledge of community service pathways to enhance client care.
• Assess and treat concussion and mild to moderate traumatic brain injuries, including cervical spine, vestibular, and oculomotor conditions.
• Work closely with general practitioners, specialists, and the wider multidisciplinary team for coordinated care.
• Design and implement individualized community rehabilitation programs focused on enhancing independence and functional ability.
• Facilitate home and community-based functional rehabilitation services effectively.
• Contribute to pain management through interdisciplinary teamwork, promoting behavioral change, pacing techniques, and education on self-management strategies rooted in biopsychosocial approaches.
• Perform functional needs assessments and recommend services that foster client independence.
• Demonstrate a strong understanding of ACC service entitlements and effectively connect clients to appropriate support networks.
• Produce detailed, high-quality clinical reports justifying recommended interventions and care plans.

Qualifications and Experience

• Possess relevant occupational therapy qualifications along with professional registration and a valid practicing certificate.
• Bring a minimum of two years’ experience in community rehabilitation settings.
• Exhibit advanced clinical reasoning skills adaptable to a wide variety of client requirements.
• Demonstrate excellent written and verbal communication abilities with clients, families, and stakeholders.
