Role Overview
The Nurse Navigator plays a vital role in coordinating healthcare services specifically for pediatric patients. This position requires working closely with a diverse care team to manage patient care throughout different healthcare settings, ensuring timely interventions and addressing any barriers faced by families.
Key Responsibilities
- Coordinate pediatric patient care across various healthcare environments.
- Encourage and facilitate early intervention strategies to benefit patient outcomes.
- Analyze clinical and utilization data to assess the effectiveness of care provided.
- Serve as a primary communication link between healthcare providers, clinics, and outside facilities.
- Exercise independent nursing judgment while following age-appropriate care guidelines.
- Collaborate effectively with support services and multidisciplinary teams.
- Engage in problem-solving and process enhancement efforts to optimize patient care.
- Adapt to both structured protocols and flexible working environments.
Qualifications and Requirements
- Possession of a nursing diploma from an accredited program is required; a bachelor's degree in nursing is preferred and mandatory in some locations.
- A minimum of two years of clinical nursing experience is desirable.
- Active registered nurse license in the state of practice is mandatory.
- Commitment to ongoing competency evaluations and maintaining certification standards.
