Job description

Role Overview

The Nurse Navigator plays a vital role in coordinating healthcare services specifically for pediatric patients. This position requires working closely with a diverse care team to manage patient care throughout different healthcare settings, ensuring timely interventions and addressing any barriers faced by families.

Key Responsibilities

  • Coordinate pediatric patient care across various healthcare environments.
  • Encourage and facilitate early intervention strategies to benefit patient outcomes.
  • Analyze clinical and utilization data to assess the effectiveness of care provided.
  • Serve as a primary communication link between healthcare providers, clinics, and outside facilities.
  • Exercise independent nursing judgment while following age-appropriate care guidelines.
  • Collaborate effectively with support services and multidisciplinary teams.
  • Engage in problem-solving and process enhancement efforts to optimize patient care.
  • Adapt to both structured protocols and flexible working environments.

Qualifications and Requirements

  • Possession of a nursing diploma from an accredited program is required; a bachelor's degree in nursing is preferred and mandatory in some locations.
  • A minimum of two years of clinical nursing experience is desirable.
  • Active registered nurse license in the state of practice is mandatory.
  • Commitment to ongoing competency evaluations and maintaining certification standards.
