Hospital Pharmacist

Role Overview

The Hospital Pharmacist is responsible for dispensing medications accurately, ensuring correct dosage, and providing pharmaceutical care to patients within the hospital setting. This includes interpreting physician orders, monitoring patient reactions to medications, and collaborating with the healthcare team to develop tailored treatment plans.

Key Responsibilities

• Dispense prescription drugs to patients accurately while verifying proper dosage and administration.
• Review and interpret doctors' prescriptions to confirm correctness and suitability.
• Monitor patients' responses to medications and modify treatment plans when necessary.
• Provide drug-related information and training to both medical staff and patients.
• Conduct medication reconciliation during patient admission and discharge processes.
• Manage pharmaceutical inventory, ensuring medicines are stored under appropriate conditions.
• Work closely with healthcare professionals to formulate personalized patient care regimens.
• Participate in clinical rounds alongside physicians and healthcare providers.
• Engage in clinical research and trials as required.
• Oversee the activities of pharmacy technicians and other support personnel.
• Ensure adherence to hospital policies and regulatory requirements regarding pharmacy practices.
• Maintain detailed patient records and document outcomes of medication therapies.

Required Qualifications and Skills

• Possess advanced qualifications in pharmacy practice or clinical pharmacy.
• Prior experience in hospital or acute care pharmacy environments.
• Proficiency using MARG pharmacy management software.
• Familiarity with hospital electronic health record (EHR) systems.
• Strong analytical thinking and problem-solving capabilities.
• Excellent communication and interpersonal skills.
• Ability to work efficiently in a high-pressure, fast-paced environment.
• Demonstrated commitment to ongoing professional growth and education.
• Must hold a valid Registered Pharmacist (RPh) license.
