Clinical Documentation Associate (Freshers)

Role Overview

We seek meticulous and process-oriented professionals to assist US healthcare providers with clinical and administrative tasks. This position requires managing Electronic Health Records (EHR), preparing charts, supporting medical documentation, handling document triage, and facilitating healthcare data migration. This role is ideal for fresh graduates or early-career individuals aspiring to establish a career in the US healthcare sector.

Primary Responsibilities

• Review, organize, and upload patient medical records into EHR systems.
• Prepare charts for physicians ahead of patient consultations.
• Assist in uploading clinical notes for upcoming appointments.
• Manage patient appointment creation and updates within healthcare platforms.
• Perform fax triage, document classification, and indexing tasks.
• Support electronic health records migration and data abstraction projects.
• Verify patient demographics and clinical data accuracy.
• Ensure precision while handling sensitive medical information.
• Collaborate with quality assurance and operations teams internally.
• Strictly adhere to HIPAA guidelines and client confidentiality policies.

Desired Skills and Qualifications

• Proficient English communication skills, both verbal and written.
• High attention to detail and documentation accuracy.
• Basic understanding of human anatomy and medical terminology preferred.
• Competent computer usage and typing abilities.
• Availability to work night shifts aligned with US time zones, if required.
• Fast learner with the ability to adapt to healthcare operational workflows.
• Familiarity with EMRs/EHRs is an advantage.

Educational Eligibility

Applicants should have qualifications such as Pharm D, B.Sc in Life Sciences, B.Pharmacy, M.Pharmacy, or M.Sc in Life Sciences.

Shift Details

Day shift from 10:00 AM to 7:30 PM.
