Role overview

Habit Health is looking for a driven Case Manager who is confident building relationships and supporting recovery outcomes. In this position, you will oversee a caseload of work-related and non-work-related injury claims, working alongside clients, injured workers, healthcare professionals, and ACC to support effective rehabilitation and a smooth return to work.

What the role involves

You will take ownership of injury claim management from beginning to end, making sure all services are delivered in line with ACC requirements, client agreements, and quality expectations. The role calls for close coordination with multiple stakeholders, informed guidance, and a strong focus on helping people achieve the best possible recovery results.

Key responsibilities

  • Handle a portfolio of workplace and non-workplace injury claims.
  • Organise rehabilitation support and drive successful return-to-work outcomes.
  • Develop and maintain productive relationships with clients, injured employees, medical providers, and ACC.
  • Work within ACC legislation, service contracts, audit requirements, and internal quality processes.
  • Share timely progress updates and reporting with clients and relevant stakeholders.
  • Deliver high-quality customer service while respecting the Code of Claimant Rights.
  • Look for ways to enhance service delivery and help support business growth.
  • Promote continuous improvement, professional learning, and strong health and safety practices.

What you need to bring

The ideal candidate combines solid technical understanding with excellent relationship-building ability. You should have a background in case management or a related health discipline, along with strong communication, coordination, and organisational skills.

Candidate profile

  • At least 2 years of experience in case management or comparable experience, or more than 5 years in a relevant health field.
  • Good working knowledge of ACC legislation and rehabilitation pathways.
  • Familiarity with ACC Accredited Employer Programme (AEP) standards.
  • Relevant tertiary study in rehabilitation, health, business, or a similar area.
  • Strong communication, negotiation, and stakeholder management skills.
  • Well-developed planning, prioritisation, and time management abilities.
  • A client-focused approach with a strong commitment to positive outcomes.
  • Ability to work independently as well as contribute effectively in a team setting.
