Across Gloucestershire we have had major investment in our Home First and Reablement Team and as a result of this we have an opportunity for a Band 6 Physiotherapist to join our Team and be a key part of developing our therapy led service in Cheltenham. Our Team is based in Cheltenham, and we work closely with the GP practices in the Primary Care Networks, alongside our Adult Social Care colleagues and voluntary and Independent sector organisations within the locality.

We can offer you the opportunity to work in a very supportive integrated team, with access to training, regular supervision, annual appraisal and the autonomy to be a lead professional in our new service delivery.
Being a car driver is essential to the role.
In line with the Government hospital discharge and community support policy, and the discharge to assess model, our Home First service is a time-limited short-term assessment service for people in their own homes to facilitate rapid discharge from Acute and Community hospitals.
We also provide Reablement to patients in the community, to prevent hospital admission and to facilitate maximum independence, working on jointly agreed goals, promoting personalised care.
