Job Title:** Registered Nurse I**Job Specialty:** Home Health**Job Duration:** 13 weeks**Shift:** Day Shift, 5×8-Hour (08:00 – 17:00)**Guaranteed Hours:** Not Guaranteed**Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience**License:** Must have a valid nursing license in hand before starting**Certifications:** CPR, BLS**Must-Have:**- Proficiency in OASIS and HomeCare HomeBase EMR, used within the last 6 months- Reliable vehicle, valid driver’s license, and auto insurance- Willingness to travel up to 75 miles from the home clinic- Ability to handle a variable mileage radius of 50-75 miles, which may involve longer drives between patients- Acceptance of all patients scheduled regardless of distance**Job Description:**- Provide, coordinate, and direct home nursing care according to physicians’ orders based on agency policies and procedures through the competent application of the nursing process.- Responsible to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and attend periodic office meetings.- Manage an average of 5-8 patients per day, with varied types of cases, including Start of Care, Wound Care, and Follow-up Visits.- Complete all patient documentation within 24 hours of the appointment.- Participate in weekend rotation for on-call duties, once every five weeks (Friday 5 pm to the following Friday 8 am).- Attend clinic meetings weekly.- Coverage will be assigned in specific county territories.- Maintain proficient and timely charting standards.- Engage in a team-oriented approach to support patient care within designated areas.
