Job Summary

The Clinical Manager is responsible for ensuring the coordination and management of

patient care in a home health setting. This role involves overseeing the delivery of quality

care, supervising clinical personnel, and ensuring compliance with organizational

standards and healthcare regulations. This role is responsible for leading and inspiring

their teams as well as clinician retention.

Work Environment: The Clinical Manager operates in a dynamic environment, balancing

administrative responsibilities with field-based interactions. This role requires flexibility,

adaptability, and a commitment to supporting the delivery of exceptional patient care.

Essential Job Functions/Responsibilities

  • Support and Education: Educates and supports clinicians to promote effective performance and the delivery of high-quality home care services. Provides assistance and guidance to clinicians during operating hours as needed.
  • Recruitment and Orientation: Assists in the screening, interviewing, and hiring process for new personnel. Participates in the orientation and training of newly hired staff to ensure they are well-prepared for their roles.
  • Standards and Compliance: Ensures adherence to accepted professional standards and principles. Consistently follows and enforces organizational policies and procedures to set an example for employees.
  • Leadership and Alignment: Leads the team in alignment with the Agency’s Mission, Vision, and Values. Promotes a culture of accountability and continuous improvement.
  • Clinician Assessment: Conducts ongoing assessments of clinicians to evaluate their understanding and compliance with policies and procedures. Utilizes ridealongs, elevate calls, and daily touchpoints as tools for evaluation and feedback.
  • Community Engagement: Participates in public relations and community activities to enhance the Organization’s visibility and reputation as a valuable member of the healthcare delivery system.
  • Performance Monitoring: Conducts regular 1:1 meetings and ride-alongs with clinicians. Provides quarterly SHP scorecard reviews with clinicians and continuous education to ensure clinical excellence.
  • Field Support: Performs weekly field visits with clinicians to mentor, guide, and support their professional growth and clinical expertise.
  • Technology Promotion: Encourages the use of patient and physician portals, use of EMR system, and clinician scheduling tools to enhance communication and care coordination. Promotes Telehealth visits to ensure appropriate visit utilization is being achieved.
  • Additional Responsibilities: Undertakes other duties and responsibilities as delegated by the Director of Clinical Services.

Qualifications

POSITION QUALIFICATIONS

  • Active RN license in the state of practice.
  • Minimum of 2 years of experience in home health or a related field.
  • Strong leadership and management skills.
  • Excellent interpersonal and communication abilities.
  • Knowledge of healthcare regulations and home health standards of care.
  • Proficient in using electronic medical records (EMR) and other healthcare

technologies.
