Collaborative Care Model (CoCM)
CoCM stands for Collaborative Care Model. It is a team-based approach to mental health that involves collaboration between primary care providers, behavioral health coordinators, medical assistants, and psychiatric providers to deliver integrated and comprehensive care to individuals with mental health conditions. The goal of CoCM is to improve access to mental health services through community-based recourses, enhance patient outcomes, and provide more holistic and coordinated care.
Behavioral Health Medical Assistant Responsibilities
- Thorough chart preparation before psychiatric appointments, which entails compiling accurate information such as current medication, hospitalizations, allergies, family medical history, etc.
- Documenting patient encounters, treatment plans, and clinical outcomes in electronic health records (EHR) or other healthcare information systems.
- Supervise Curogram and uphold ongoing communication with patients via this platform to attend to any requirements or concerns.
- Consistently monitor provider schedules to ensure that they are fully booked each day.
- Review provider progress notes to assist with scheduling patient for follow ups, labs, testings, etc.
- Ensure patient records are current and accurate.
- Communicate with pharmacies, healthcare providers, and insurance companies as necessary to facilitate seamless patient care.
- Support Behavioral Health Coordinators by aiding in scheduling, patient communication and provider communication when needed.
- Adhere to ethical and legal standards, including maintaining patient confidentiality and upholding professional boundaries in the delivery of mental health services.
Behavioral Health Medical Assistant Requirements:?
- Must have certification as a Medical Assistant.
- Proven experience as a Medical Assistant.
- Strong organizational, communication, and computer skills are important for success in these roles.
- Must be able to work as part of a multi-disciplinary team
- Knowledge of health and patient care regulations preferably geared towards behavioral health
- Excellent communication skills
- Strong ethics
- Willingness to continue gaining knowledge and clinical experience
