Position Summary: Provides outreach and community-based therapeutic services and engagement referrals to local children, adolescents, and individuals with special needs. Coordinates and provides information, educational materials, resources and consultation to individuals and their families. Builds rapport with local agencies, schools and community partners to provide case management and care services.
Qualifications: High school diploma or equivalent. One year related work experience preferred. Knowledge of local community resources. Valid Driver’s License with a good driving record.
Preferred Qualification: minimum 1-year experience working with children and adolescents and/or 1-year experience working with special needs population.
Special Skills, Knowledge and Abilities: Outstanding organizational skills with good time management practices. Able to work independently with an “off-site” supervisor. Ability to work in partnerships and groups effectively and cooperatively. Ability to speak publicly with members of community organizations and schools. Demonstrates understanding and patience in working with participants coming from vulnerable background populations. Ability and willingness to educate others and coordinate care services. Ability to learn and utilize computer software.
Essential Functions/Job Duties
- Reports to work on a daily basis as scheduled and performs necessary program tasks.
- Completes assessments to determine client physical and emotional needs.
- Participates in the development and maintenance of the client’s treatment plan.
- Periodically reviews and updates client’s record and documents case management activities according to State standards and recipient needs.
- Advocates for and links client with needed services and resources.
- Participates as a team member and contributes to the enhancement of the department.
- Works with the clients, families and natural support system, in client’s natural environment.
- Presents and travels to necessary locations to accommodate needs of targeted populations.
- Performs other related duties as assigned by supervisor.
Work Environment/Physical Demands: While performing the duties of this job, the employee regularly works in varying environments. The employee may be located in office, remote, or within the community. The noise level in the work environment varies by location. While performing the duties of this job, the employee is required to sit, stand, walk, carry up to 10 pounds, and drive. Specific vision abilities required by this job include close vision and distance vision. The employee may be exposed to various medical and health conditions. The employee is required to be able to hear and communicate verbally and in writing.
Position Type, Expected Hours of Work and Travel: This is a full time, 40-hour position. Work schedule is Monday – Friday, with hours and days varying by event, worksite location, and community resource schedules. Some weekend and evenings as necessary. Local travel and commute required.
