Duties

The Case Manager is a registered nurse responsible for overseeing patient care in line with physician orders and state regulations. Reporting to nursing leadership, key duties include:

  • Developing and evaluating patient-centered care plans.
  • Managing a group of patients and coordinating with healthcare providers.
  • Conducting initial evaluations and ongoing assessments of nursing needs.
  • Advocating for patients and educating families on care strategies.
  • Documenting patient progress and participating in quality assurance activities.

Qualifications include a nursing degree, state licensure, and preferably one year of home care experience. The position offers a full-time schedule with various benefits.
