Job Description

PQC is seeking a Transition Coordinator who will work as part of the Indiana Pre-Employment Transition Services (Pre-ETS). The Transition Coordinator will work at Union City High School. The goal of the Pre-ETS initiative is to improve post-school outcomes of students with disabilities ages 14 to 22. These services shall be provided during the school day, after school hours or weekends and/or in the summer months in various settings including schools, businesses, community organizations, and other stakeholder locations. Services may include self-advocacy, career exploration, job readiness, job shadowing, situational assessments and work experience opportunities with area employers.

Responsibilities

  • Utilize a screening assessment to gauge student workplace readiness to move forward in pursuing competitive employment.
  • Assist students in identifying barriers; and deliver, document, and analyze the effectiveness of tools and techniques to assist students.
  • Assist students to identify resources within their family, community and school system to help prepare for employment, education and training.
  • Provide goal-oriented transition services to promote student’s personal and professional development through a coordinated set of activities designed to offer exposure to appropriate decision-making and conflict resolution techniques.
  • Assist students to identify activities, experiences and opportunities that will increase their competencies in the areas of self-advocacy and leadership.
  • Provide financial capability training for students to acquire, develop, and improve money management skills.
  • Assist students in exploring local, available, affordable, and accessible transportation options; and will provide instruction in the use of public transportation to access work and the community.
  • Maintain Student records, including student progress reports with emphasis on challenges and achievements.

Qualifications

  • Previous experience working with a variety of individuals with disabilities.
  • Must have strong oral and written communication skills.
  • Ability to pass a mandatory background screening.
  • Proficient with use of various technology and tools.
  • Valid Driver License and at least 18 years of age.

Duty Hours: primarily 7:30 am- 4:30 pm

Shift: Fulltime

Pay: $22.00/ hour
