Salary: $4.00 – $8.00 per hour based on experience

Schedule: Monday to Friday, 9:00 AM – 5:00 PM, Eastern Standard Time

Job Description:

As a Patient Care Coordinator, you will closely monitor patients’ health indicators, interact with patients, and complete medical documentation. This role requires a excellent communication skills and a passion for assisting others.

Requirements:

  • Strong interpersonal and communication skills to interact with patients, families, and healthcare providers.
  • Knowledge or ability to quickly learn healthcare terminology and patient care procedures.
  • Capability to maintain confidentiality in all patient interactions and documentations.
  • Proficient in basic computer software (e.g., Microsoft Office, EMR systems).
  • Strong organizational skills and the ability to manage multiple tasks simultaneously.

Key Responsibilities:

  • Monitor Health Indicators: Regularly track and evaluate patient health indicators, ensuring any changes in status are promptly addressed and communicated.
  • Monthly Patient Interaction: Conduct live, ongoing monthly interactions with patients to assess their health, answer questions, and provide support.
  • Chronic Illness Education: Educate patients on strategies for managing chronic illnesses and improving overall health outcomes.
  • Maintain Open Communication: Foster clear, open lines of communication with patients to support their needs and address concerns in a timely manner.
  • Health Summary Management: Monitor and update patient health summaries, ensuring the accuracy and confidentiality of all medical information.
  • Health Education: Provide patients with information about their health indicators and what they mean, empowering them to make informed decisions.
  • Complete Medical Documentation: Accurately complete encounter forms and medical forms as needed, ensuring compliance with regulatory requirements.
  • Track Healthcare Goals: Monitor and track patient progress toward their individual healthcare goals, providing encouragement and adjustments to care plans as necessary.

Successful candidates for this role will be rewarded with the following benefits:

  • A total of five paid vacation days per year (additional days can be taken unpaid).
  • A 5% raise per year guaranteed (additional raises or bonuses may be given for outstanding performance).
  • A paid 15-minute break for every 4 hours of work.
  • Healthcare benefits after 2 months of full-time work.

* Vacation days increase by 1 vacation day per year. I.e. after one year of work, the total paid vacation days on year 2 would be six, on year 3 seven, etc.
