Job Overview

In the capacity of RN Case Manager, you will be instrumental in delivering a positive experience for clients and their families. You will act as the primary contact for clients, guiding them through the inquiry process and managing their ongoing care needs.

Key Responsibilities

  • Respond promptly to inquiries and conduct in-person nursing assessments to introduce clients to our services.
  • Develop tailored care plans, match caregivers with clients, and ensure caregivers are well-oriented to meet client needs.
  • Maintain proactive communication with all stakeholders involved in the care process.
  • Adapt care plans based on the evolving needs of clients.
  • Build strong and trusting relationships with clients and their families to ensure satisfaction.
  • Cultivate relationships with community referral sources and creatively promote our services.

Required Skills

  • Bachelor’s or Associate’s Degree in Nursing from an accredited institution.
  • Active Registered Nurse (RN) license in Maryland.
  • Strong verbal and written communication abilities.
  • High attention to detail with excellent organizational skills.
  • A collaborative and team-oriented mindset.
  • Commitment to delivering exceptional customer service and care.

Qualifications

  • Minimum of two years nursing experience.
  • Previous case management experience is advantageous, though not mandatory.
  • Availability to travel within the Baltimore County, Harford County, and Baltimore City areas for client assessments and visits.
