Role and Facility Information

We are seeking primary care physicians to staff for the innovative Stabilization Services in Nanaimo that will move into the New Central Nanaimo Urgent and Primary Care Centre that officially opens in May 2025.

The service provides primary care follow-up to unattached patients who present with new, or unstable, diabetes, hypertension, heart failure, COPD and new / suspected cancer diagnoses.

The service was developed through a Nanaimo community medical response to help address the needs of unattached patients, and to support the NRGH ED and acute care units and has been extended to support unattached patients who present to Oceanside Urgent Care, Ladysmith Urgent Care, Medical Arts Urgent Care and Urgent Medical Assessment Clinic.

The service provides short term access to follow-up primary care that aims to reduce the risk of admission or re-admission for unattached patients. This much-needed service will support patients with their immediate post-discharge primary care needs, and the team will work to link patients to other services for on-going care and support. Patients referred to the service will be varied from simple follow-up to patients with more complex care needs requiring further follow-up and connection to other services. Access to Stabilization Services requires a referral from approved referring agents and care will be provided by booked appointments only.

The role will offer flexible hours, team-based care (RN, SW, MOA, CNL, Medical Lead and Management) to offer comprehensive patient support. The Patient’s Care is provided by the Family Practitioner during the clinic hours with the support of the multi-disciplinary team to provide ongoing patient support throughout the duration of the patient’s involvement with the clinic.

This service provides a great deal of opportunity to influence clinic design, operations and culture.

Compensation: Group Physician Contract with a rate of $198.48 (with additional evening increase of $25/hr starting at 18:00)
