JOB TITLE: Patient Navigator
REPORTS TO: Practice Manager
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:
- Greet, interview and register patients using eClinical Works
- Assist new patients with the registration process
- Check necessary documents for identification and insurance eligibility
- Orient patient and distribute patient bill of rights
- Act as information clerks to make proper referrals as needed
- Responsible for fee collection and daily cash reconciliation
- Make appointments
- Identify and link eligible patients and their families to primary care, mental health, restorative dental services, substance abuse treatment, specialty care and other required services through the LIFQHC.
- Facilitate and monitor scheduling and arranging of medical and dental appointments for patients and their families. Provide follow up services for newly diagnosed individuals who require additional services.
- Responsible for linking patients to health plans, navigate with health plans, obtain pre-authorizations and referrals for patients as needed.
- Assist with distribution of materials relating to food stamps, 340b and other related patient care related material.
- Track referrals to ensure patient access and support patients in overcoming barriers to care.
- Track specialty visits and send consults to referring providers.
- Take messages and return phone calls for patient appointment.
- Confirm patient appointments.
- Medical records maintenance.
- Sort faxes and distribute to staff accordingly.
- Other related duties as assigned.
QUALIFICATIONS:
- High School Diploma or equivalent required
- Professional, well organized and detail oriented
- Able to perform prolonged mental effort, complex Multi – tasking, organizing & priority setting
- Able to use standard office equipment (phone, fax, copy machine, scanner, email/voicemail
- Knowledge of Microsoft Office
- Experience in health care
- Bilingual in English and Spanish/Creole preferred
- Physical ability to perform functions of job (lifting, mobility and manual dexterity)
SALARY: Commensurate with experience
