Position Summary
The Nurse Navigator plays a key role in delivering exceptional patient care by coordinating services for individuals with complex and chronic conditions. Through care management and education, this position ensures patients adhere to prescribed therapies, improving health outcomes and quality of life.
Required Knowledge, Skills & Training
- Must be an excellent communicator and problem-solver

- Fully competent in MS Office (Word, Excel, PowerPoint)
- Current RN License in good standing (Compact licensure a plus but not required)
- 5+ years of clinical experience in healthcare, including hospital, home health, pharmaceutical or biotech
- 2-5+years of case management
Essential Job Duties
- The Nurse Navigator is responsible for participating in one-on-one communications with patients and their caregivers
- Provide direct educational training and support to patients and caregivers about prescribed medication
- Will manage patient cases across regions as coverage and volume requires
- Communicate insurance coverage updates and findings to the patient and/or caregiver
- Review and educate the patients and/or caregivers on financial assistance programs that they may be eligible for
- Coordinate logistical support for patient to receive therapy and manage their disease
- Collaborate with Home Office, HUB Case Coordinators, and Field based teams
- Collaborate with Reimbursement Manager teams to troubleshoot and resolve reimbursement-related issues
- Engage with patients and provider to ensure appropriate support is being given on an individualized basis
- Provide patient-focused education to empower patients to advocate on their behalf
- Develop relationships and manage multiple and complex challenges that patients and caregivers are facing
- Ensure compliance with relevant industry laws and company policies
- Participate in and complete required pharmacovigilance training
- Comply with all relevant industry laws and company policies
