Job Summary: The Population Health Outreach Specialist’s primary objective is to work with the Population Health team on value-based program projects. This role involves providing support for client implementation, primary care provider (PCP) assignment/reassignment, and handling outreach to health plan partners. This position supports the Population Health team by engaging in strategic outreach to Community Health Alliance (CHA) patients. It involves organizing, engaging, and tracking outreach efforts that promote health and wellness in the community. This position must demonstrate exceptional customer service skills and professionalism. This position is required to be bilingual (Spanish speaking).

Essential Duties And Responsibilities

  • Responsible for creating and receiving both data reports that reflect the strategic need for outreach.
  • Assists the Population Health team in working outreach reports.
  • Outreach engagement must be performed in a consistent, kind, compassionate, and respectful manner.
  • Scheduling patients appropriately.
  • Tracking of all outreach progress as advised by the Population Health team.
  • Providing outreach reporting to the assigned Population Health team staff.
  • Provide resource assistance during outreach calls (as needed).
  • Responsible for collecting, submitting, and tracking primary care provider (PCP) forms.
  • Communicate with health plans regarding special projects, including PCP forms.
  • Consistently promote health and well-being with a focus on preventative health.
  • Must demonstrate strong ability to prioritize and ensure ongoing efforts toward team and project short-term and long-term goals to ensure the completion of the associated projects.
  • Must practice exceptional communication skills and exhibit professionalism when directly engaging with the community.
  • Keep accurate records and maintain patient confidentiality.

Required Knowledge, Skills & Abilities

Education and Experience: High School Diploma or GED required. One year of relevant experience is preferred. Must be able to read, write, and speak Spanish fluently.

Work environment: Indoor environment

Physical demands: Ability to maintain driver’s license and to travel throughout the community as well as between clinic sites as needed. Ability to sit or stand. Ability to express or exchange ideas by means of spoken word to impart oral information to co-workers or patients and to convey spoken instructions to others accurately, loudly or quickly. Ability to hear and understand verbal communication.
