What You Will Do
- Conduct proactive outreach via phone and email to support new enrollments into Caregiver Coaching program and follow-up to expeditiously advance qualified individuals through the eligibility and enrollment process.
- Provide program information specific to various health plans, educating on the value of the Careforth’s services and thoughtfully answering questions.
- Work across multiple computer systems accurately and efficiently to enter documentation of all activities while adhering to quality standards, maintaining confidentiality, and observing HIPAA requirements.
- Offer excellent customer service using empathy, inquiry, and advocacy.
- Perform the intake process, gathering and documenting information pertaining to both the caregiver and care recipient.
- Support caregiver onboarding to drive early engagement; carry out Careforth app provisioning, orientation, and guidance to complete initial account setup.
- Collaborate and communicate cross functionally with the Care Coaching team to ensure seamless case handoffs.
- Perform inbound and outbound communication tasks (calls, emails, text) to support Careforth enrollment goals.
- Provide recommendations for process improvements to optimize workflows that support growth.
- Participate in special projects and perform other duties as assigned.
What You Will Bring
- 2+ years high-volume customer service or inside sales experience, preferably in healthcare.
- Strong attention to detail and experience with data integrity in a regulated environment.
- Exceptional active listening and communication skills with a focus on empathy; multilingual skills a plus.
- Demonstrated ability to multitask across multiple systems and easily learn new process flows.
- Proficiency with MS Office and Excel; experience with Salesforce and case management systems preferred.
