Position Summary
A Case Manager provides continuity of care, with the goal of permanent placement, for children through a case management model that includes developing, expanding, accessing and linking resources in the community to the needs of the child throughout the child’s experience in the system, while documenting progress.
What You Will Do
- Conducts assessments and prepares treatment plans.
- Ensures psycho-social well-being.
- Assures clinical contract and regulatory compliance goals are met.
- Provides case management as appropriate.
- Offers counseling and support services to truant clients and families.
- Assists children in completing education through eligibility determination, intake, assessment, evaluation, referral, transportation, and monitoring school attendance.
- Completes comprehensive documentation related to counseling services.
- Records contact notes, assessments, testing, screening, intake forms, interview notes, correspondence, and statistics.
- Schedules and conducts client visits at convenient locations.
- Arranges informal and formal contacts with individuals relevant to specific clients.
- Maintains contact with visiting teachers to initiate Truancy Referrals from the School Board.

Requirements
WHAT YOU NEED:
Education & Experience: Bachelor’s Degree; 4+ years post Bachelors experience directly in human services, preferably in counseling OR case management, or master’s degree; 0-3 years post master’s clinical, counseling or case management experience.
Physical Requirements: Ability to travel regionally on a regular basis and statewide as needed; Ability to bend, lift and carry 30 pounds.
