  • The OB Coordinator position will provide support to the below five clinics.

Job Summary

The OB Coordinator is accountable for all primary functions of OB tracking, which begins when a patient initiates pregnancy care and continues through the first six weeks of postpartum support. The OB Coordinator will review daily reports, identify the patient’s needs according to protocols, and provide outreach. The OB Coordinator will work closely with the patient and clinical team members to ensure that all clinical measures and care are addressed in a timely manner in alignment with Patient Centered Medical Home Practices.

Preferred:

  • Bilingual skills.

Education

  • High school graduate or equivalent.
  • Associates degree in health-related field, or in lieu of degree, a combination of equivalent education and work experience.

Preferred:

  • Graduate of an accredited Medical Assisting program.

Experience

  • Experience in health care or social services including knowledge of OB care (2 years).

Preferred:

  • Experience in FQHC healthcare setting.

Credentials

  • Basic Life Support for Healthcare Providers (CPR/AED) certification – must be obtained within 90 days of employment.

Other

  • Driver’s license with the State of Washington.
  • Motor vehicle insurance liability policy, a certificate of deposit, or a liability bond to the required limits.

Job Specific Functions/Performance:

  • Responsible for screening patients for appropriateness of care with coordination of external referrals, as needed.
  • Coordinates patient assignment to provider panels.
  • Responsible for performing initial prenatal intake to include past medical history.
  • Assists patients with management of their pregnancy care related needs and social support services including, referrals, scheduling, and communication of provider determined plan of care.
  • Reviews educational handouts with patients and routes questions to appropriate staff members, as needed.
  • Attends OB related team meetings.
  • Registers patients for hospital delivery and schedule C-section procedures as needed.
  • Maintains statistical data on OB program including but not limited to delivery information, transfer of care, and all UDS/HEDIS quality measures.
  • Monitors patient adherence to plan of care, providing outreach as needed.
  • Prepares monthly reports for supervisors and providers.
  • Data entry and patient chart review.
  • Faxes OB records to external organizations, obtaining release of information and outside records.
  • Adheres to attendance standards in order to perform the job functions for daily operations and/or continuity of patient care.
