Job Description
- NOTE** Internal applicants are required to submit a Letter of Introduction, an updated Resume and any additional required documents (new or in need of an update) not on file. Substitutes must submit ALL required documents. EXPERIENCE: One (1) year of paid experience performing health related services, public contact in a physician’s office, hospital or related setting preferred Training in substance abuse and crisis intervention desired EDUCATION: Certificate Copy (Licensed Vocational Nurse, EMT, Certified Medical Assistant, or Certified Nursing Assistant) preferred but NOT required)

- CPR/First Aid Certification (Must have valid CPR & First Aid Certificate)
- Driver’s License Copy (Must be valid)
- Letter of Introduction
- Letter(s) of Recommendation (Must submit two (2) letters dated within the past twelve (12) months.)
- Proof of HS Graduation (Or equivalent)
- Resume
