The Intake Registered Nurse (RN) is responsible for conducting comprehensive assessments of patients during the intake process. This role involves evaluating patients’ medical histories, determining their care needs, coordinating with healthcare providers, and facilitating the smooth entry of patients into the healthcare system. The Intake RN ensures that patients receive timely and appropriate care, and maintains accurate records of all interactions. Key Responsibilities:Patient Assessment: Conduct initial assessments of patients, including reviewing medical histories, current health status, and psychosocial needs.Care Coordination: Collaborate with physicians, specialists, and other healthcare professionals to develop and implement patient care plans.Documentation: Maintain accurate and up-to-date records of patient interactions, assessments, and care plans in the electronic health record (EHR) system.Patient Education: Provide information and education to patients and their families about treatment plans, procedures, and available services.Referral Management: Facilitate referrals to appropriate healthcare providers and services as needed, ensuring continuity of care.Compliance: Ensure that all intake processes comply with relevant regulations, policies, and standards of practice.Communication: Serve as a liaison between patients, families, and healthcare providers, addressing any concerns or questions.Crisis Management: Handle urgent or emergency situations as they arise, providing appropriate care and coordinating with emergency services if necessary.Quality Improvement: Participate in quality improvement initiatives and contribute to the development of best practices for patient intake processes. Qualifications:Education: Associate’s or Bachelor’s degree in Nursing (ADN or BSN) from an accredited nursing program.Licensure: Current Registered Nurse (RN) license in NY.Experience: Previous experience in a clinical setting is preferred; experience in intake or case management is a plus.Skills:Strong assessment and critical thinking skills.Excellent communication and interpersonal skills.Proficiency in using electronic health record (EHR) systems.Ability to manage multiple tasks and prioritize effectively.Compassionate and patient-centered approach to care. Physical Requirements:Ability to lift, carry, and assist patients as needed.Ability to stand and walk for extended periods.Good manual dexterity for operating medical equipment and completing documentation. Working Conditions:Work may be performed in a clinical or office setting. Salary: $95,000 – $110,000/annually Schedule: Monday – Friday, 8:30 AM – 5 PM Benefits: Health Dental Vision PTO
