LVN Home Health Intake Specialist
The LVN Home Health Intake Specialist is responsible for managing the intake process for patients in a home health agency, ensuring timely and accurate admission, assessment, and care coordination. This position combines nursing expertise with administrative duties to streamline patient intake, provide initial assessments, and facilitate smooth transitions into care, all while maintaining a high standard of quality and patient safety.
Qualifications:
- Current and active LVN license in the state of Texas.
- Minimum of 1-2 years of experience in a healthcare setting, preferably in a home health intake or admissions role.
- Experience with HCHB EMR and patient documentation is preferred.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare providers.
- Excellent organizational and time-management abilities.
- Ability to handle confidential and sensitive information with discretion.
- Knowledge of insurance verification and authorization processes is a plus.
