Job Overview

City Net is seeking a dedicated and passionate Outreach Case Manager II (CMII) to join our team in Riverside, CA. This vital role involves providing case management and supportive services to individuals and families facing street-level homelessness. The CMII will work actively to connect unsheltered neighbors with sustainable housing solutions while encouraging them to evade citations and code violations. By meeting clients in their current situations, the CMII will assist them in achieving their personal goals through a housing-focused approach that emphasizes long-term planning and connection to essential supportive services, including healthcare and counseling.

Key Responsibilities

  • Deliver comprehensive case management and support to individuals and families experiencing homelessness.
  • Facilitate connections between clients and sustainable housing, as well as appropriate supportive services.
  • Collaborate with various community partners, including nonprofits, government entities, law enforcement, and healthcare providers, to deliver necessary assistance effectively.
  • Uphold City Net’s Housing First and Harm Reduction service models in all interactions.
  • Collect and report quality data in a timely manner to ensure effective service delivery.
  • Operate in diverse outdoor environments, engaging directly with clients in their neighborhoods and informal encampments.
  • Provide flexible availability, including evenings and weekends, to meet client needs.

Required Skills

  • At least 1 year of case management experience within relevant populations, such as those affected by homelessness or related challenges.
  • Strong interpersonal and analytical abilities to foster professional relationships.
  • Excellent verbal and written communication skills in English.
  • Proficiency in Microsoft Office Suite and a solid understanding of data collection and reporting requirements.
  • Ability to work both independently and collaboratively within a team setting.
  • Demonstrated empathy, self-motivation, and adaptability in diverse situations.

Qualifications

  • Preferred bachelor’s degree in Social Work, Human Services, Sociology, Psychology, or a related field.
  • Proven experience in building and maintaining relationships with various stakeholders in the community.
  • Capacity to manage multiple projects efficiently while maintaining high standards of creativity and strategic thought.
  • Flexibility to work varied hours, including early mornings, evenings, and weekends as required.

Compensation And Benefits

  • Pay Range: \(23.53 – \)25.53 per hour
