Case Manager
Job Description
We are seeking a Case Manager for the Bucks County Department of Corrections in Doylestown, PA, to coordinate and implement the behavioral health program, focusing on after-care planning. This full-time position requires 40 hours on-site, Monday to Friday, with an hourly rate of $23.00-25.00.
- Monitor and coordinate clinical services in line with NCCHC/ACA/MCCS Standards.
- Assist in suicide prevention and coordinate admissions to behavioral health facilities.
- Document services in electronic medical records and develop discharge plans for seriously mentally ill patients.
- Collaborate with community providers for post-release resources.
- Participate in committees and in-service education programs while maintaining confidentiality.
Requirements include a Bachelors Degree in a related field, one year of clinical experience preferred, and strong communication skills.
