Schedule: Full-time

Shift: Monday – Thursday 8-5pm / Friday 8:30am – 3pm

The Role You’ll Contribute

The RN Patient Care Coordinator is responsible for coordinating the care of patients within their specialty. Responsible for ensuring the full continuum of patient care is being met.

  • Coordination: May be Responsible for the following:
    • New patient consults and patient work up coordination
    • Appointments and scheduling with continuation of care
  • Patient Care: May be Responsible for the following:
    • Initiates, facilitates and monitors the development of patient care pathways that identifies critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance and promotion for a specific patient group.
    • May assist in donor search, selection and coordination
  • Communication:
    • Promotes an environment of open communication as demonstrated through interaction with patients, staff, providers, faculty, administration and outside facilities.
    • Utilizes positive problem-solving skills and conflict resolution to assess and diffuse difficult situations.

Minimum Qualifications

The expertise and experiences you’ll need to succeed:

  • Associate’s Degree in Nursing (ASN)
  • 1 year of RN experience
  • Basic Life Support
  • Florida State RN License

Preferred Qualifications

  • Care Coordination Experience
  • Relevant specialty experience
  • Bachelor’s Degree in Nursing (BSN)
