Resident Care Coordinator Overview & Responsibilities

Responsible for assisting the Health and Wellness Director in coordinating services for resident care. Reports to the Health and Wellness Director. Duties may include caregiving, training, transportation arrangements, supply management, chart management, service plan management, audits, incident reports, miscellaneous forms management and other duties related to resident care. Works in cooperation with the Nurses, Administrator and other appropriate staff.

Job Scope:

This job has recurring work situations with occasional variations from the norm. Duties involve a high degree of complexity and need for attention to detail due to the nature of all the life care services this entails. Duties are performed under the indirect supervision of the Nurses and Administrator with established procedures and guidelines.

Resident Care Coordinator Responsibilities:

  • Assist in mentoring and supervision of care staff as needed to ensure the service plan is followed as instructed
  • Maintain all necessary record-keeping on a computer
  • Participates in on-site training and ongoing education as requested
  • Keeps Nurses/Administrator informed of any issues or concerns that arise
  • Maintain schedules for resident needs (shower schedule, contact list, etc.)
  • Weekly audits of resident needs

Resident Care Coordinator Qualifications

  • Ability to understand and work under a variety of situations in a positive, constructive manner
  • Ability to be organized, set and meet deadlines
  • Ability to perform many daily tasks via computer, i.e., emails, spreadsheets, etc.
  • Excellent interpersonal skills with those inside and outside the organization including residents, their families, volunteers, management, civic organizations, community, and agencies
  • Maintain cooperative working relationships with those contacted in the course of work activities
  • Knowledge of principles and practices of Residents’ needs
  • Skill in problem-solving, and the ability to remain poised in emergencies
  • Required to receive the annual Flu vaccine, excluding religious or medical exemptions.
