This position provides referral coordination for the Mobile Medical Unit (MMU) as part of Health Care for the Homeless (HCH) through the Pinellas County Department of Health and in partnership with Direction for Living’s (DFL) Housing First Division. The MMU provides primary care to homeless individuals with DFL, providing psychiatric, counseling, and case management services in the field as part of the Mobile Medical Unit and at Bayside Clinic. This position is co-located at the Bayside Clinic.
Must have experience in the mental health or social services field.
This role encompasses the coordination of intake processes, scheduling assessments, and determining program eligibility to ensure timely and appropriate access to services for clients. The Referral Coordinator serves as the primary point of contact for the referral sources and clients, tracking referral status, and facilitating communication among stakeholders. The Referral Coordinator also plays a key role in educating potential clients, community partners, and providers about available services.
Education Experience/Requirements
- High School Diploma, with at least 2 years of experience in mental health, social services, or healthcare coordination.
- Familiarity with referral systems and intake processes is a plus.
- Experience with Avatar required
- Knowledge of Insurance and funding sources
