An exciting opportunity has arisen for suitably qualified Band 4 Assistant Practitioner to join our established team providing care to an individual with complex health needs within their own homes.

You will have already acquired or currently be working towards a minimum of a level 4 qualification in Health and/or Social Care and have extensive experience as a Band 3 HCSW.

An exciting opportunity has arisen for suitably qualified Band 4 Assistant Practitioner to join our established team providing care to individual with complex health needs within their own home in Monmouthshire.

As an Assistant Practitioner you will work with a wide MDT under the delegation of a team of Registered Nurses. You will utilise and develop a wide variety of skills including airway management, care of tracheostomy & ventilator and be able to meet the emotional, physiological, and clinical needs of individuals.

The role is challenging but rewarding and you will be fully encouraged and supported with your professional development. The Care at Home Team has a dynamic internal education team with a focus on professional development.

The ability to drive patient vehicles and the ability to travel between sites in a timely manner is essential.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications

Essential criteria

  • Hold or working towards a minimum of a Level 4 QCF qualification / HNC/ HND/ foundation degree in Health and/or Social Care
  • Hold a minimum of a NVQ/QCF Level 3 degree in Health and/or Social Care

Experience

Essential criteria

  • Sufficient years experience working as a band 3
  • Previous experience working within the NHS
  • Able to supervise the work of band 2 and 3 healthcare support workers and work as part of a team.

Desirable criteria

  • ECDL
