Home Health Registered Nurse (RN)
We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. As a Home Health RN, you will play a crucial role in supporting individuals recovering from illness or surgery, managing chronic conditions, and ensuring they receive the personalized care needed to thrive in a home setting. You will collaborate with interdisciplinary teams to create and implement care plans, educate patients and families, and promote overall health and wellness in the home environment.
Key Responsibilities
- Patient Care: Provide comprehensive nursing assessments, administer medications, and perform nursing procedures as prescribed by the physician.
- Care Planning: Develop, implement, and revise individualized care plans based on patient needs, collaborating with the physician, family, and other healthcare professionals.
- Monitoring and Evaluation: Monitor patient progress, vital signs, and condition changes. Report any significant changes to the physician and adjust care plans accordingly.
- Patient Education: Educate patients and their families on proper care techniques, medication management, lifestyle=””>Documentation: Maintain accurate and timely patient records, documenting assessments, interventions, outcomes, and progress in compliance with regulatory and agency standards.
- Collaboration: Work closely with other healthcare team members, including physical therapists, social workers, and physicians, to ensure coordinated and comprehensive care delivery.
- Emergency Response: Respond to medical emergencies in the home setting, providing immediate care and intervention as needed.
Qualifications
- Licensure: Active and unrestricted Registered Nurse (RN) license in IN.
- Experience: Minimum of 5 years of experience as an RN, preferably with experience in home health, geriatrics, or a related field.
- Skills: Strong clinical assessment, critical thinking, and problem-solving skills. Ability to work independently and manage a diverse caseload.
- Certifications: BLS (Basic Life Support) certification required; additional certifications in Home Health (e.g., CHPN) a plus.
- Personal Attributes: Excellent communication skills, both verbal and written, with the ability to build rapport with patients and families. Strong organizational and time-management skills.
