Looking for CNA/HHA Wednesdays and Fridays 10am to 8pm.

Responsibilities

  • Provides direct patient care within the patient’s home, under the direction of the RN, and according to the Aide Plan of Care.
  • Appropriately reports changes in the patient’s condition to the RN/the care team and documents pertinent information.
  • Practices in accordance with accepted infection control principles. Provides a clean, safe, and comfortable environment.
  • Provides necessary assistance to patients with light housekeeping, meal preparation, proper nutrition, and adequate fluid intake.
  • Promotes positive, supportive, and respectful communication with patients/clients, caregiver(s), families, and other employees.
  • Provides assistance to patients with safe transfers and ambulation per the Agency policies.
  • Completes yearly education hours.

Minimum Qualifications

  • Active Certified Nursing Assistant certificate, one year’s experience in direct client care, or has provided home health services within previous 24 months.
  • Current CPR certification
  • TB testing
  • Dependable transportation

This role is a 1099 contract opportunity, with care being provided at the patient’s residence in accordance with a physician established care plan.
