Job Summary: Provides integrated behavioral health and substance use services within primary care settings, focusing on prevention, intervention, and recovery.

Major Duties

  • Conduct screenings and assessments for behavioral health and substance use needs.
  • Create and implement treatment plans, modifying as necessary.
  • Engage patients and families through outreach and referrals.
  • Consult with medical providers for coordinated care.
  • Document services per policy and maintain compliance.

Skills And Abilities

  • Culturally relevant service provision and addressing healthcare disparities.
  • Adaptability to changes in healthcare practices.

Education/Experience

  • Masters degree in Social Work, Counseling, or related field required.
  • Licensure in Maryland required; LCSW-C or LCPC preferred.
  • Two years of relevant experience preferred.
  • Multi-lingual candidates preferred.
