Job Summary: Provides integrated behavioral health and substance use services within primary care settings, focusing on prevention, intervention, and recovery.
Major Duties
- Conduct screenings and assessments for behavioral health and substance use needs.
- Create and implement treatment plans, modifying as necessary.
- Engage patients and families through outreach and referrals.
- Consult with medical providers for coordinated care.
- Document services per policy and maintain compliance.
Skills And Abilities
- Culturally relevant service provision and addressing healthcare disparities.
- Adaptability to changes in healthcare practices.
Education/Experience
- Masters degree in Social Work, Counseling, or related field required.
- Licensure in Maryland required; LCSW-C or LCPC preferred.
- Two years of relevant experience preferred.
- Multi-lingual candidates preferred.
