About The Position

  • Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5×8-Hour (08:00 – 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year of Adult Home Health or Hospice experience **License:** Must have a valid license; no exceptions **Certifications:** CPR, BLS **Must-Have:** – Proficiency in HomeCare HomeBase and OASIS used within the last 6 months, or completion of a retraining course – Reliable transportation, current driver’s license, and agency-required liability insurance **Job Description:** – Provide, coordinate, and direct home nursing care according to physician’s orders based on agency policies and procedures through the competent application of the nursing process. – Responsible for home visits, periodic office meetings, and supervision of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. – Expected to manage an average of 5-10 patients daily, which equates to roughly 6-8 points per day. – Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. – Territory coverage includes travel up to 75 miles from the home clinic, with mileage reimbursed at the standard rate. – Expected to maintain weekly productivity points of 30, equating to 40 hours per week. – Must be open to traveling up to 75 miles and accept every patient on the schedule. – Completion of documentation within 24 hours of patient appointments is required. – Participate in weekend call rotations and be open to floating to nearby clinics if needed to meet financial needs. – Attend any mandatory orientation and comply with all agency and client-specific requirements.
