Remote RN Case Manager | WFH Opportunity

We are seeking a dedicated and compassionate Registered Nurse (RN) for the position of Case Manager in a remote capacity based in Maryland. Key Responsibilities

As an RN Case Manager, your primary responsibilities will include:

• Conducting warm, in-person nursing assessments for new clients and responding to inquiries.
• Developing tailored care plans and overseeing the onboarding process for clients.
• Collaborating with caregivers to ensure clarity and confidence in implementing care plans.
• Maintaining ongoing communication with clients, families, and caregivers for monitoring and adjustments to care plans.
• Building trusting relationships that promote the growth of services within the local community.

Required Skills

To excel in this position, you will need:

• A Bachelor’s or Associate’s Degree in Nursing from an accredited institution.
• An active Registered Nurse license in Maryland.
• A minimum of two years of nursing experience, with prior case management experience being advantageous.
• A proactive attitude towards community relationship building and service excellence.
• Excellent written and verbal communication skills, along with strong attention to detail and organizational abilities.
• The ability to travel within Baltimore County, Harford County, and Baltimore City for client assessments.
• COVID-19 vaccination (with accommodations available for medical or religious reasons).

Qualifications

• Active Registered Nurse license in Maryland.
• At least two years of nursing experience.
