Remote Field Care Manager RN | WFH

We are seeking a dedicated and enthusiastic Registered Nurse (RN) to fill the role of Field Care Manager. This remote and field-based opportunity based in Charlotte, NC allows you to play a crucial role in the lives of individuals with long-term care needs while enjoying the flexibility of a work-from-home arrangement.

Key Responsibilities

• Manage a diverse caseload of healthcare members requiring long-term care support.
• Perform regular assessments through both home visits (80% travel) and telephonic follow-ups (20% remote).
• Collaborate effectively with members, caregivers, and healthcare providers to create and refine personalized care plans.
• Ensure the seamless delivery of services and monitor the effectiveness of care.
• Coordinate necessary referrals to facilitate smooth transitions in care without interruptions.
• Collaborate with discharge planners, physicians, and other healthcare professionals to ensure comprehensive care plans covering both acute and long-term services.
• Support members in navigating complaints, appeals, and other concerns regarding their care.
• Maintain meticulous documentation and assessments utilizing electronic health records.

Required Skills

• Active RN license in North Carolina, in good standing.
• Proficient in Microsoft Office and Microsoft Teams.
• Valid North Carolina driver’s license, as travel is an essential component of this role.

Qualifications

• Bachelor’s degree in nursing.
• A minimum of two years’ experience in care management.
• Experience in service coordination, preferably within a managed care setting is desirable.
• Bilingual proficiency in English and Spanish is highly preferred.
