Job Description
Key Responsibilities
- Manage a diverse caseload of healthcare members requiring long-term care support.
- Perform regular assessments through both home visits (80% travel) and telephonic follow-ups (20% remote).
- Collaborate effectively with members, caregivers, and healthcare providers to create and refine personalized care plans.
- Ensure the seamless delivery of services and monitor the effectiveness of care.
- Coordinate necessary referrals to facilitate smooth transitions in care without interruptions.
- Collaborate with discharge planners, physicians, and other healthcare professionals to ensure comprehensive care plans covering both acute and long-term services.
- Support members in navigating complaints, appeals, and other concerns regarding their care.
- Maintain meticulous documentation and assessments utilizing electronic health records.
Required Skills
- Active RN license in North Carolina, in good standing.
- Proficient in Microsoft Office and Microsoft Teams.
- Valid North Carolina driver’s license, as travel is an essential component of this role.
Qualifications
- Bachelor’s degree in nursing.
- A minimum of two years’ experience in care management.
- Experience in service coordination, preferably within a managed care setting is desirable.
- Bilingual proficiency in English and Spanish is highly preferred.
